IMPORTANT – NEW PRACTICE SOFTWARE UPGRADE!
Throughout 2015, we’ve seen our practice grow and change to meet the expanded needs of our patients. These changes have included new staff, treatment rooms, and as of January 4, 2016, an upgraded Practice Management and Medical Record system called Nextech. Please be aware that at your next visit you will be asked by a CCSD staff member to re-scan previously captured information and to complete ALL NEW forms including an updated Financial Policy and Credit Card Authorization Form. So that we may expedite your registration and use of this new system we ask you to please bring the following items to your next visit (even if you have been seen at our office before):
– Your current insurance card
– Credit, debit or HSA card (in your name)
– Valid form of photo ID

The office will be closed on Monday January 4,2016 for an all staff training day. When calling the Practice, please follow the prompts and DO leave a message. We will get back to all voice mails and emails as soon as we can.
