The new year often means a change of insurance provider or policy. If you have new insurance, or want to know if your policy is in network, please call the office in advance of any forthcoming visit so we can run a check on your eligibility and benefits.
You need to bring a copy of your current insurance card to all appointments, otherwise may risk being considered self-pay for the day. Please be aware that we do require all patients to leave a credit card on file, and have found this to be a more convenient and private option for you to pay your bill, wherever you are. With the increase in high-deductible health plans, If you have any questions please don’t hesitate to speak with our billing department, 312-245-9965, option 4.
Lastly, we’d like to remind all patients that our Practice has operated a last minute cancellation and no show fee since 2016. If you need to cancel or reschedule any appointment, we do kindly ask for more than 24 business hours notice, otherwise a fee may be applicable. As a popular Practice, we operate this policy to ensure all patients have an equal and fair opportunity to see their chosen provider.
