It’s happening!! In a few short days we will be moving into our new office space at 515 N. State Street, Suite 900! We are extremely excited and ready for all our patients to see the new, state-of-the-art office! Dr. Jacob has put so much time into creating an office that staff, patients & guests will all enjoy.
With a big move, unfortunately, come some necessary interruptions. We will be closing the office and shutting down our phone systems on Thursday, April 4th at noon. We will be open again and taking calls on Monday, April 8th at 8 am. We apologize for any inconvenience this may cause our patients and ask for your patience during this time. You can either leave us a voicemail or email [email protected], however we will be unable to answer your emails or return calls until Monday, April 8th. We kindly ask that you call us for your appointments and/or prescription refills before noon on Thursday, April 4th or on Monday, April 8th.
